Mesotherapy for hair


Mesotherapy is a method highly useful in ailments and diseases where direct application of medicine or medicated substance does not give a satisfactory result. It is also used to give enhanced results than local applications or internal medicines, although a combination of all the above treatment might be suggested as per requirement to give better results. When mesotherapy is done on the scalp or for hair related issues it is called as mesohair. It involves a series of injections given directly into the scalp with the help of a very fine needle; this is usually painless and is done under local anesthesia. Mesohair is very effective in conditions like alopecia (baldness), telogen effluvium (hair loss) and to promote hair growth. The medicine to be delivered by the mesotherapy technique is chosen after a detailed consultation, with consideration of the hair problem or requirement. It is usually planned in multiple sessions depending on the severity and intensity of the problem, and visible results are usually seen approximately after a month as hair growth and its healing requires time. This period might vary from person to person.


PRP Hair


What is PRP? How is it done?


PRP stands for Platelet Rich Plasma. This is obtained from the patient’s blood and then processed to extract the plasma containing a high amount or concentrate of platelets along with healing factors, nutrients, and other essential components. After extraction of PRP is done, it is used fresh on the patient. As it is a concentrate only small amounts are injected at multiple sites on your scalp, this is then followed by a micro-needling session to increase its efficacy.

How does PRP help?


The human blood mainly comprises of plasma and the blood components like red blood cells, white blood cells, platelets, nutrients, etc. Blood flows through the body and nourishes every tissue, but sometimes despite the nourishment provided aging signs start to appear due to multiple issues. Also due to toxin build-up or improper blood circulation the amount of nutrients required does not reach the tissue; thus leaving the hair undernourished. To treat such condition nutrition and healing factors are to be provided in high concentration, here PRP provides a solution.


PRP also promotes hair rejuvenation, enriches scalp with nutrients, and enhances the volume of hair by thickening of hair strand. The procedure is done under strict hygiene and sterile environment, also proper anesthesia is given locally to the patient making the treatment almost painless.

What is Androgenic alopecia or AGA?


Androgenic alopecia (AGA) is a condition affecting both men and women. It is also called as the aging hair loss, yet this condition is commonly seen in young individuals in a high incidence rate. Reasons might vary on individual basis but the major trigger factor is the sensitization of the follicle to androgen, specifically DHT or dihydrotestosterone a derivative of testosterone. This leads to pathology where the hair follicle receives altered or less nutrition due to DHT sensitization. This is seen in crown region of scalp, or in simple words the area covered if a crown is placed on the head.


AGA in men


Usually men suffering with AGA are noted with hair line recession from the temporal and occipital region or the head triangle corners. The side corners and the center of scalp presents diffuse loss of hair. The condition begins from early 20’s and progress slowly as age increases. If the progression is at a higher rate it is also observed that near to complete baldness is reached till the age of 30 or 35.


AGA in women


In women AGA is usually a lesser prevalent condition as compared to men. Also the hair loss pattern is different and mostly diffuse hair loss is seen all over the scalp, with prominence depending on hair style habits. The condition is triggered mostly due to underlying physical conditions like ovarian cyst, thyroid imbalance, multiple pregnancies with short periodic lapse, use of birth control pills and menopause.


Treatment of AGA


Treatment of the condition is mostly to conserve the hair follicle by providing better nutrition and growth factors at the scalp. More specifically PRP or Platelet Rich Plasma therapy is very useful and if coupled with Mesotherapy, it gives even better results in some cases. PRP is derived from the patient’s blood making it acceptable to the scalp as its own body components are injected in concentrate form. PRP is rich concentrate of growth factor, healing factors, platelets, etc. Mesotherapy is a process of injecting small amounts of nutrition in the form of serum on the scalp, thus adding to the volume of hair by thickening the hair strand as a benefit of better nutrition.

Both PRP and Mesotherapy are followed by a session of micro-needling. This induces a false alarm of injury drawing the healing factors along with collagen and fibrin to the scalp. This helps in rejuvenation of the scalp and also increases the scalp thickness, providing with a rich blood supply. Both of the therapies give good results in patients if done with planned multiple sessions, for at least 3 to 6 months depending on the stage of hair loss.


Role of oral medications


Along with the above mentioned series of treatments it is also equally important to give oral medications like nutritional supplements and herbal formulations to counter aggressive aging of the patient. Herbal medications help to counter aging with almost no side-effects. Herbal oils like Malatyadi oil, Kuntalkanti oil, Neelibhringadi oil, Bhringamlyakadi oil, etc. are helpful in preventing scalp dryness and nourishing the scalp externally.

Other treatments like Nasya – nasal medication with Prapundrikadi oil or other such medicated oil or ghrita (clarified butter) are recommended based on individual body type. Shirodhara – rhythmic oil flow over fore head with medicated oil or butter milk helps in reducing stress and stress induced triggers for AGA. Detoxification with the help of Panchkarma helps to get rid of toxin barriers and enhances the tissue with overall increased nutrition at cellular levels.

It is observed that the benefits of PRP and Mesotherapy are indeed fantastic but they last no longer than 1 or 2 years; thereafter the same process has to be repeated to counter lack of nourishment and aging. Here if Ayurvedic therapies along with herbal internal medications are coupled along with the sessions of PRP and Mesotherapy it gives a better and comparatively long lasting results.



Telogen Effluvium



What is Telogen effluvium (TE)?


Telogen effluvium (TE) is one of the most common diagnoses in a skin and hair clinic. In simple language it can be described as a change in the number of hair follicles that are growing hair; the live or growing hair follicles are reduced in number while the shedding follicles are increased causing a visibly increased hair fall. Fortunately, the condition is treatable and is not permanent.

This condition affects the top of the scalp most often. Rarely TE causes the hairline to recede, also unlikely that you’ll lose all of your hair. Although in some severe cases, TE can cause hair in other areas to fall out, like your eyebrows and pubic region.


What happens in Telogen effluvium?


In a healthy person's scalp, about 85-90% of the hair follicles are actively growing hair (anagen hair) and about 10-15% are resting hairs (catagen + telogen phase hair). The average growth period of an anagen hair about 4-5 years or more rests for about 4 months (catagen phase). After this process, the telogen phase or the falling/shedding phase hair begins and is then followed by anagen phase as the new hair pushes the old once out; thus completing the hair cycle.

Around up to 100-125 hair is normal to lose in a day including the hair fallen on the comb, brush, in the basin, or on the pillow, as a result of the normal scalp hair cycle.

Due to multiple triggers, as many as 70% of the anagen hairs can be precipitated into telogen, thus reversing the usual ratio. Typical triggers include:

Childbirth: postpartum hair loss. This can resolve after a few months or transition into female pattern alopecia.

Physical trauma such as an accident causing blood loss

Physiological neonatal hair loss

Acute or chronic illness, especially if there is fever

Surgical operation

Long-term use of medicines (eg, hypertensive, etc.)

Psychological stress

Weight loss, unusual diet, or nutritional deficiency (eg, iron deficiency)

Certain medications

Endocrine disorders (eg, hypothyroidism, hyperthyroidism)

Oral contraceptive pill

Recurrent overseas travel resulting in jetlag

Skin disease affecting the scalp (eg, psoriasis)

Excessive sun exposure.



How is Telogen effluvium (TE) diagnosed?



Telogen effluvium is usually diagnosed by its clinical features.

Hair thinning involves the entire scalp +/- loss of other body hair.

Examination shows diffuse thinning without focal areas of total alopecia and short hairs of normal thickness.

A gentle hair pull test reveals an increased number of hairs; most are telogen with a typical epithelial sac.

A tricoscopic examination can help confirm the diagnosis; ruling out the thickness of hairs along with short hair strongly suggests telogen effluvium.

Never needed is a scalp biopsy but in cases with other underlying pathologies such as psoriasis or chronic dermatitis, it may be suggested.
